Please follow the instructions below to submit your contracts with FFSG:    

Step 1: Use the following link to start the process: Register and Submit Contracts With FFSG 

Step 2 A: New Agents will need to fill out all fields and select the “Submit Form” at the bottom of the page.

Step 2 B: Current Agents with active Medicare/ACA contracts will need to fill out all required fields, the contracts you would like with FFSG, the List of States you would like to be contracted in and your Signature.  After your information has been entered, select "Submit Form".

Step 3: You’ll receive a confirmation email letting you know we've received your request.
